Rehabilitative Services

Our interdisciplinary team works together to provide a comprehensive array of therapy services. Our Physical, Speech and Occupational Therapists use a variety of treatment modalities such as exercise, balance and mobility training, along with assistive devices to help you recover from the from a stroke, orthopedic surgery, amputation, cardiac condition, traumatic brain injury, visual or hearing loss.We strive to maximize your function within your home setting.

Joint Replacement

  • More than a half million Americans will need total knee or hip replacement surgery this year. Our clinicians provide a comprehensive assessment, consult with your physician, and utilize a variety of treatment modalities to help safely you regain your mobility and independence at home. Our specialized program coordinates rehabilitative care prior to surgery through our Pre-Op program.
  • Pre-Op: Prior to surgery our therapist will come out to your home to evaluate your post- surgical needs and make recommendations for any equipment or assistive devices you may need once you transition back to home. In addition, our nurses and therapist will provide a great deal of patient education to ease your stress before surgery.
    PT/INR: Our clinical staff will provide PT/INR monitoring for patients taking anticoagulation therapy in conjunction with their skilled nursing or PT visit.

Stand Strong/Fall Prevention

Stand Strong! Is our unique fall prevention program dedicated to keeping our patients as safe as possible within the home setting. Since 1 in 10 patients coming home from the hospital will fall within the first month. Family Care Visiting Nurse clinicians take our fall prevention program very seriously and every patient is entered into the program regardless of diagnosis. Your fall risk is objectively evaluated each and every visit from all of the staff providing your care. We will work with you and your family in how to prevent falls within the home and teach you how to stay as safe as possible. Your safety is our goal. If adaptive equipment is required, we will stay with you through the process and make sure that you are satisfied.

Pediatrics

Family Care Visiting Nurse has a highly trained group of therapists that thoroughly address the special needs of children. We understand the unique musculoskeletal, neurologic, psychological and sensory issues that children can face. As a result, our team works closely with the family and medical team to help the child meet their goals. No matter what the age or the diagnosis we are there to help that child move through their developmental milestones.

PT/INR

Coumadin® (warfarin sodium) has proven invaluable in treating a variety of serious conditions (such as atrial fibrillation, hip/joint replacement, heart valve replacement, stroke, etc.) and in reducing the incidence of complications including stroke, systemic embolism and death in persons with atrial fibrillation.

The CLIA-waived testing system uses thromboplastin reagents to measure patients’ PT/INR levels. The test, which requires a fingerstick instead of a blood draw, is performed in the comfort of the patient’s home.
Results are available in about four minutes.

To be effective, anticoagulation therapy requires consistent, reliable PT/INR monitoring.

The PT/INR System allows the patient’s physician to receive critical blood level results to adjust the patients anticoagulant level immediately as needed.
