FamilyCare Visiting Nurse and Home Care Agency, LLC (“FamilyCare”) is a home health and visiting nurse agency serving patients and families across Connecticut. We deliver skilled nursing, therapy, home health aide, and related in-home services in accordance with applicable state and federal health care standards, including HIPAA.

This Policy does not describe how we treat protected health information (PHI) we collect in the course of providing care.
