Make a Confidential Report
- FamilyCare Visiting Nurse and Home Care Agency, LLC is an organization built on strong values of responsibility, integrity, and compassionate care. Our Code of Business Conduct and Ethics sets general guidelines for delivering home health services and operating our agency with the highest standards of ethics, patient safety, and regulatory compliance.
- FamilyCare is committed to an environment where open, honest communication is the expectation, not the exception. We want our patients, families, employees, and clinicians to feel comfortable approaching their supervisor, clinical manager, or agency administration whenever they believe a violation of policy, patient rights, or standards of care may have occurred.
In situations where you would prefer to submit a report anonymously or in confidence, you are encouraged to use the hotline below or submit an anonymous report. You may report concerns related to our Code of Business Conduct and Ethics, patient care, HIPAA or privacy matters, billing integrity, or workplace conduct. You are also welcome to request guidance on policies and procedures or share positive suggestions and stories about the care we provide. - The information you provide will be sent to our Compliance Officer on a completely confidential — and, if you choose, anonymous — basis. You have our guarantee that your comments will be heard and reviewed.
To Make a Report
You may use either of the following two methods to submit a report:
- Submit a digital report, either anonymously or not, by visiting our Contact page and marking the message as a confidential report.
OR - Call us toll-free within the United States at (800) 946-6331.
After you complete your report you will be assigned a unique case code, and one of our compliance officers will begin reviewing your concern.
Our Compliance department is NOT a 911 or Emergency Service:
Do not use this page to report events that present an immediate threat to life, health, or property. Reports submitted through this service may not receive an immediate response. If you or a patient require emergency assistance, please contact 911 or your local authorities right away.
